501(c)3 Charter Renewal Form

This form is an annual report as required by the IRS for PCAI to maintain its “Group Status” and continue to provide you the 501(c)3 tax-exemption for your ministry. Even if your chartered certificate renewal date is for this year, you are still required to provide the following information for our records.

Important Legal Information

  • You may use this form for your financial report or, if you use QuickBooks or similar program, you can attach a Profit & Loss Statement for the financial portion only.
  • Your ministry is required to file a 990 form with the IRS, as well as attach a copy here.
  • Check with your Secretary of State office to verify that your state incorporation is current and active. Renewal frequency varies from state to state. Without valid state incorporation, your federal incorporation (501(c)3 status) becomes invalid. You may or may not receive notice from your Secretary of State that it is due as it is your responsibility to handle this.

FORM DEADLINE: March 28th of each year

Please note: We cannot extend the renewal past March 28th due to the IRS deadline requirement of May 15th. If you are late sending us the report necessitating us to file an amendment, you will be required to pay a late fee of $150. Once your report is received in our office and approved, your charter will be renewed, and a letter of good standing will be mailed to you within 30 days. We also file a report with the IRS confirming your status.

    Be sure to keep records and receipts of all expenses listed above.
    IMPORTANT NOTICE: You must keep a complete record of all people and organizations who contrinute to the ministry and how much they gave for your records. Don't forget to issue a "contribution statement" to contributors for tax purposes.

          IMPORTANT NOTICE: IRS 990 Form Filing is due May 15th. Your ministry is required to file a 990 form with the IRS. Contact our office if you have questions about the 990. It is due by May 15th to file with the IRS and a copy must be sent to our office for your file. *If your ministry gross income was $50,000 or less, you file an online 990-N. (Per your request, our charter department will file this online form for you.) *If your ministry gross income is $50,000 up to $200,000, you file a 990EZ. *If your ministry gross income is $200,000 or more, you file a 990 Long Form. Please contact your tax professional concerning these forms.
          Issue W-2s and/or 1099s to those who received wages or to whom you paid contract labor. If you need help concerning these forms, contact your tax professional or local IRS office. You are to require anyone you pay contract labor to fill out a W-9 and keep in your records.
          NOTE: You must keep complete records of all people and organizations who contribute to the ministry and how much they gave. Don't forget to issue contributions statements to those who donated to your ministry.
